In order to enroll your child in KidsCare you must complete & sign ALL forms, unless other-wise stated. A registration fee of $135 (per first two children of a family) is required. You can conveniently print out the forms or the entire KidsCare brochure (with forms included) using the links below and complete them at your leisure.
